Available Positions

HCHV Case Worker

Non-exempt Fulltime, $30.60 per hour
Northport

PURPOSE
Provide direct and indirect services to Veterans living at the shelter on the grounds of the Northport VA. Improve the quality of life, enhance social functioning, and empower program participants to identify permanent housing solutions. Aide in contributing to building long-term sustainability within the lives of our program participants. Connect program participants with the services and resources necessary to help achieve and maintain their goal of independent living

REQUIREMENTS
– LMSW (Licensed Master of Social Work) or LMHC (Licensed Mental Health Counselor)
– Prior case management experience preferred.
– Experience using data entry platform, HMIS, preferred.
– Experience, preferably in a residential program, serving the homeless or – similar population.
– Experience working with Substance Abuse and Mental Illness, preferred.

ESSENTIAL JOB FUNCTIONS
– Completes an initial needs assessment screening and acts upon critical needs appropriately and immediately.
– Completes a request for Military Service Record (DD214 form) upon intake.
– Observes the principle of confidentiality and adheres to the company’s Code of Ethics
–  Orients clients to the shelter program’s requirements and consequences
– Assures that basic nutrition and hygiene needs are being met.
– Develops an Independent Living Plan, tracks progress and evaluates outcome with goals for permanent housing identified.
– Monitors progress towards goals weekly sessions.
– Refers clients to appropriate resources to assist with meeting goals.
– Maintains client case records to include ILP, and correspondence, progress towards goals and documentation of any ILP violations.
– Maintain HMIS database with updated progress notes and running log of interactions.
– Report critical incidents immediately to the appropriate program director.
– Collects data necessary to meet funding requirements and statistical.
– Maintains strict confidentiality of all client and staff information.
– Participates in regular fire and other emergency drills with the house manager, support staff and clients.
–  Demonstrates ability to use panic buttons when necessary for emergency situations.
– Provides regular reports to the program director on the progress of each case including noticeable changes in appearance and behavior.
– Follow-up with maintenance for all repairs and replacements.
– Prioritizes residents who are inappropriately referred to relocate within forty-eight hours or the following business day.
– Performs other record-keeping functions required by the HCHV Contract.
Works with the VA liaison to meet client goals.

Veteran Housing Case Worker

Non-exempt Fulltime, $25.75 per hour
Amityville

PURPOSE 
Provide direct and indirect services to individuals, groups, and communities with the objective of achieving mutual client system and worker goals. Improve the quality of life, enhance social functioning, and empower program participants and their families. Aide in contributing to building long-term sustainability within the lives of our program participants enrolled in Permanent Housing and the VA’s Grant and Per Diem Program. Connect program participants with the services and resources necessary to help achieve and maintain their goal of independent living.

REQUIREMENTS
– 2 years of case management experience or bachelor’s degree in Mental Health Counseling or Human Services related field or a minimum.
– Experience, preferably in a residential program serving the homeless or similar population.
– Experience working with Substance Abuse and Mental Illness preferred.
– Experience using data entry platform.

ESSENTIAL JOB FUNCTIONS
– Attend weekly case conferences with Senior Case Worker.
– An assessment is to be completed to identify the individual needs of the resident within 30 days, diagnosis to be identified.
– Conduct weekly walk-throughs of assigned UVBH residences.
– Conduct monthly fire drills with the house manager.
– Conduct monthly site inspections.
– An independent living plan will be developed and implemented within the first two weeks of admission into our program. The I.L.P. is intended to track progress and evaluate outcomes.
– Make referrals and provide resources to address the individual needs of the residents. Explore the residents’ eligibility for referrals or information; this should be given high priority.
– Provides periodic reports to supervisor on the progress of each case including noticeable changes in appearance and behavior.
– Provide weekly In’s & Out’s reports to Senior Case Worker.
– Update rents charge form as needed.
– Review monthly print out and submit corrections to Senior Case Worker within 5 business days.
– Review annual inspections and weekly walk-through reports to address any corrective actions.
– Performs record keeping functions required by the agency, e.g., case records to meet state standards, HMIS, and the Current Client list.
– Assist residents who are inappropriately referred to relocate within forty-eight hours.
– Rotating on-call weekends with other Case Workers.
– Maintaining a minimum of two face-to-face meetings per month with each client (GPD).

DSS Housing Specialist

Non-exempt Fulltime, $23.73
Suffolk County

PURPOSE
The Housing Coordinator works as a member of the social service team to provide Rapid Rehousing and Homelessness Prevention services to our program participants. Functions as a bridge between residents and housing providers to empower program participants to overcome barriers and meet the goals identified on their individualized housing stability plans. Identifies safe, affordable, permanent housing options for clients; fosters relationships with landlords and realtors to link each program participant with appropriate community supports to ensure their housing stability and sustainability.

REQUIREMENTS
– High School Diploma or GED Equivalency.
– Experience with homeless family services preferred, but not required.
– Demonstrable proficiency with Microsoft Office Suite required.
– Experience with electronic database systems preferred (e.g. HMIS).
– Must have a valid NYS Driver’s License.

ESSENTIAL JOB FUNCTIONS

– Assist individuals and families in identifying, applying for, and securing permanent affordable housing, including preparation for housing applications and interviews.

– Conduct comprehensive assessments to identify individual and family barriers to securing and maintaining affordable permanent housing.

–  Maintain regular contact with participants to monitor progress and collaborate on individualized stability plans for Homelessness Prevention and Rapid Rehousing services.

– Provide supportive services designed to promote housing stability, including individual and group counseling, workshops, education, referrals, and transportation assistance.

– Build and maintain strong relationships with landlords, housing providers, and community partners to support participants, while developing and sustaining a housing inventory of available units through ongoing outreach, collaboration, and networking.

– Maintain complete, accurate, and up-to-date documentation in compliance with funding requirements and agency quality assurance standards.

– Track, monitor, and report housing placement rates for all participant families.

– Coordinate the provision of Temporary Financial Assistance with third parties and agency personnel.

DSS Case Worker

Non-exempt Fulltime, $27.50
Suffolk County
Full-time onsite position, hiring for multiple locations

– Report critical incidents immediately to the appropriate Beacon House and DSS staff.
– Collects data necessary to meet funding requirements and statistical.
– Conduct weekly walk-throughs of assigned UVBH residences.
– Conduct regular fire and other emergency drills with the house manager, support staff and clients of assigned UVBH residences.
– Provides periodic reports to supervisor on the progress of each case including noticeable changes in appearance and behavior.
– Follow-up with maintenance for all repairs and replacements.
– Assist residents who are inappropriately referred to relocate within forty-eight hours. Performs other record keeping functions required by the DSS Contract, Police Activity Report, Critical Incident Report, etc.
– Works with the DSS liaison to meet client goals.
– Responsible for all aspects of the shelter assigned; this includes the physical condition of the property, the clients and their cases, the staff working within the house, the technology used by the staff within the house. This includes all computers, phones both cellular and land, laptops, and Biometric Time Clock.
– Supervise the weekly scheduling of the shift work within the shelter.
– Attend weekly meetings with Program Manager/Program Director and fellow Case Managers.
– Work closely with the Housing Specialist to locate permanent housing for clientele.
– Conduct regular meetings with caseload.
– Assist clients with setting up laundry transportation. (Motel)
– Rotating on-call weekends with House Managers and other Case Workers.
